Children's Continuing Care Nursing Service

The Children’s Continuing Care Team (CCCT) provides care to children in their homes or other community settings 365 days per year. Each child’s package is individual to the needs of the child and their family. The team compromises of 3 nurses, 1 admin assistant, 1 nursing associate and 24 nursery nurses.
A continuing care package is required when a child or young person has needs arising from disability, accident or illness that cannot be met by existing universal or specialist services alone. Some children and young people may have very complex health needs. These needs may be so complex, that they cannot be met by the services which are routinely available from GP practices, hospitals or in the community commissioned by clinical commissioning groups (CCGs) or NHS England. A package of additional health support may be needed. This additional package of care has come to be known as continuing care. Continuing Care was formally developed in Luton in 2008, prior to this Children with complex needs were cared for by other agencies which were costly to the service.
